Principal Investigator (PI) proposal questionnaire

I will be assisting you with your proposal submission to (agency name). Outlined below are a few generic questions and points to assist me with the submission of your proposal. This is a boilerplate document to get us started. Please review and answer the following questions. I believe that these questions will also help you prepare a strong, organized proposal.

  1. Is this proposal in response to a particular agency program or Request for Applications? If so, which one?
  2. If the agency is NIH, are you (PI) submitting an application/proposal where the development of model organisms is anticipated? Please state yes or no here. ___ (Yes) ____ (No)
    1. If yes, then the PI is expected to include in the application/proposal a description of a specific plan for sharing and distributing unique model organism research resources generated using NIH funding so that other researchers can benefit from these resources, OR state appropriate reasons for why such sharing is restricted or not possible. FAQs and sample sharing plans are available at the NIH Model Organism Sharing Policy page at the National Institute of Health Office of Extramural Research web site.
    2. If requesting $500,000 or more in direct costs in any year, you will need to provide a paragraph how final research data will be shared. For more information, please refer to NIH Data Sharing Policy page at the National Institute of Health Office of Extramural Research web site.
  3. Does the proposed project involve any of the following compliance issues: Animal Care and Use (IACUC), Human Subjects (HSC-L), Biohazards, Recombinant DNA or Radioactivity? Please review the policies at the Research Integrity web site for clarity.
  4. What is the "title" of your proposal?
  5. What is your proposed start and end dates?
  6. Please list the name and organization of any PI's and Co-Is for this proposal.
  7. Do you plan on using any of the following in your proposal? If so, please list names and organizations.
    1. Collaborators
    2. Subcontracts
    3. Consultants
    4. Services
    5. Participants
  8. If any other personnel will be named in the proposal, please list their names, affiliations, and corresponding roles in the proposal.
  9. Is any cost share/match planned in this budget? If yes, is it required?

    Please review the Cost share policy. If you will be requesting matching funds from KUCR, please arrange matching funds with any other units and then contact Associate Vice Provosts for Research, George Wilson (gwilson@ku.edu) or Joshua Rosenbloom (jrosenbloom@ku.edu) as soon as possible.
  10. Do you plan on identifying Haskell (HINU) in your proposal submission? If yes, please explain.

  11. Please indicate which center/department you wish to submit this proposal through.
  12. Describe what your budget will support (senior and other personnel, postdoc, GRA, UG, travel, equipment, research materials and supplies, etc.)
